    Background: India has the maximum burden of both non MDR tuberculosis (TB) and Multidrug-Resistant (MDR) TB, as per data reported in Global TB Report 2018 and tuberculosis is remains one of the most common cause of pleural effusions.Methods: This was a cross-sectional study conducted in Department of Respiratory Diseases and a total of 110 patients with pleural effusion were included in the study, which were enrolled for treatment from July 2018 to June 2019.Results: One hundred and ten patients with pleural effusion were enrolled during the study period. There were 65 males (59%) and 45 (40.9%) females.  The overall mean age for males and females were 44.4±18.84 years (35-87 years) and 38.28±17.66 years (35-87 years) respectively. Tuberculous Pleural Effusion group (TPE) seen in 82 patients. Right sided pleural effusion (69.5 %) were more common than left sided (30.4 %). In TPE group the mean pleural fluid ADA level were 86.41±38.08 IU/L (range: 14-195 IU/L). The Malignant Pleural Effusion (MPE) group included 21 patients. In MPE group the mean pleural fluid ADA level were 34.10±32.88 IU/L (range: 8-144 IU/L). The difference in pleural fluid ADA levels between TPE and MPE group was statistically highly significant.Conclusions: Tuberculous pleural effusion was the most common cause of pleural effusion in present study and observed in 74.5% cases

    CAM5.2 Expression in Metastatic Tumours of CNS: A Diagnostic Tool

    Introduction: Secondary tumours or metastases account for more than half of all brain tumours in adults. Central nervous system is most commonly a target of metastatic dissemination. The judicious use of  selected immunostains is unquestionably helpful in diagnostically challenging cases. CAM 5.2 being highly specific, is emerging as a specific marker to diagnose metastatic carcinoma.Presentation of case: Total six metastatic tumours were studied using CAM5.2. Histopathological sections of brain tissue were stained by routine hematoxylin and eosin (HE) as per standard technique. Representative sections were subjected to immunohistochemical staining with CAM 5.2. Skin biopsy act as a positive control for cytokeratin.All of the 6 cases showed positivity for CAM 5.2. CAM5.2 expression in metastatic tumours was statistically significant (sensitivity 100% 100% specificity).Conclusion: We conclude in our study that CAM5.2 was significantly associated with metastatic tumours, as they were positive using this specific marker

    Auditing of prescriptions in relation to diarrhea in children below 5 years of age: a multicenter study

    Background: This study was planned to determine the prescribing pattern of drugs in children below 5 years of age suffering from diarrhea by different categories of doctors in the city of Jaipur (Rajasthan).Methods: This observational retrospective study was conducted in the Pediatric Outpatient Department of SMS Medical College and other hospitals in Jaipur (Rajasthan). In this study, 300 prescription (10% of total prescription) of the children aged below 5 years, suffering from acute diarrhea, were randomly selected.Results: As alone, norfloxacin was noted in 49.2% prescriptions followed by ofloxacin in 24.6% out of 61 prescriptions. In combination, the most common antimicrobial (77.78%) prescribed was norfloxacin with either metronidazole or tinidazole.Conclusions: Antimicrobials should be prescribed rationally for pediatric patients suffering from diarrhea to avoid potential adverse events and increased cost of the treatment . Regular prescription audits in hospitals should be undertaken to promote rational use of drugs

    Self-supported amorphous TaNx(Oy)/nickel foam thin film as an advanced electrocatalyst for hydrogen evolution reaction

    Chemical vapor deposited (CVD) amorphous tantalum-oxy nitride film on porous three-dimensional (3D) nickel foam (TaNx(Oy)/NF) utilizing tantalum precursor, tris(diethylamino)(ethylimino)tantalum(V), ([Ta(NEt)(NEt2)3]) with preformed Ta–N bonds is reported as a potential self-supported electrocatalyst for hydrogen evolution reaction (HER). The morphological analyses revealed the formation of thin film of core–shell structured TaNx(Oy) coating (ca. 236 nm) on NF. In 0.5 M H2SO4, TaNx(Oy)/NF exhibited enhanced HER activity with a low onset potential as compared to the bare NF (−50 mV vs. −166 mV). The TaNx(Oy)/NF samples also displayed higher current density (−11.08 mA cm−2vs. −3.36 mA cm−2 at 400 mV), lower Tafel slope (151 mV dec−1vs. 179 mV dec−1) and lower charge transfer resistance exemplifying the advantage of TaNx(Oy) coating towards enhanced HER performance. The enhanced HER catalytic activity is attributed to the synergistic effect between the amorphous TaNx(Oy) film and the nickel foam

    Pulseless Right Upper Limb: An Unusual Manifestation of Invasive Pulmonary Aspergillosis in Acute Myeloid Leukemia

    Abstract Aspergillus is the most common cause of fungal pneumonia in acute leukemia patients receiving chemotherapy or undergoing hematopoietic stem cell transplantation. Despite a high index of suspicion and prompt institution of specific antifungal therapy, it causes significant morbidity and mortality in patients with hematological malignancies. It has to be differentiated from mucormycosis because the treatment differs. Histological confirmation obtained by lung biopsy is ideal, but is difficult to obtain in those patients who often have thrombocytopenia. We report a case of acute megakaryoblastic leukemia with typical manifestations of invasive pulmonary aspergillosis who developed pulseless right arm due to invasion of the right subclavian artery. When total leucocyte counts recovered, patient also developed immune reconstitution inflammatory syndrome and massive pulmonary hemorrhage, which was managed by bronchial artery embolization

    The possibility of electron attachment to the valence 3Π^{3}\Pi state of CO is examined using an {\it ab initio} bound-state multireference configuration interaction approach. The resulting resonance has 4Σ−^{4}\Sigma^{-} symmetry; the higher vibrational levels of this resonance state coincide with, or are nearly coincident with, levels of the parent a3Πa^{3}\Pi state. Collisional relaxation to the lowest vibrational levels in hot plasma situations might yield the possibility of a long-lived CO−^- state.Comment: Revtex file + postscript file for one figur
